A seventy year old individual with diagnoses inclusive of a learning disability and autism, also has multiple health needs. The gentleman lives with his wife in their own home in the community.

The gentleman requires 6 hours support weekly to support him with shopping, completing post correspondence actions, and with making and attending appointments.
