PA Support Community Access (85515) LKM

Gentleman who is 60 years old and lives alone in a rented flat in Ventnor, his daughter lives locally and he is visited weekly by his ex-partner. Diagnosed with schizophrenia, which is managed by anti-psychotic medication, he attends the depot clinic every other week and would like a PA who drives to support him with this appointment and to access the community for shopping and to attend local groups and events. The gentleman is also conscious that his diet isn’t as good as it could be, he would like to work with a carer for shopping, meal planning, preparation, and batch cooking. The gentleman is a smoker. There is the expectation that a PA would work with the care agency and other professionals in providing care.
