PA support 4 hours a week to access the community, attend appointments and maintain the home environment. £11.95 per hour

Older lady who lives in an maisonette in Cowes.

Has a diagnosis of dementia, up until recently she has been receiving support from her daughter and son to maintain her needs.

Has since lost the ability to process and sequence instructions, without encouragement she will not get motivated to get out of bed, eat or drink.

She would benefit from having increased support to maintain her needs which will also support daughter who is not able to sustain he level of support she has recently been providing to Mum.

Requires support with well being and attending health appointments, without support in these tasks she is at risk of malnutrition and dehydration, risk of unintentional self-neglect, risk to safety, risk to physical health.
