Lady with physical and mental health and this affects her functioning. There are times when she struggles to attend to self- care, due to her mood and pain. For healthy life and functioning she would like support in her own home and to access the community x1 week.

She is exhausted from the challenges she has faced in her life. She still suffering from poor mental health, with low mood, anxiety, and flashbacks within the context of her trauma. Some risk of relapse in her mental state due to the stress of having been homeless over the last two years. She has recently just been housed and there is still uncertainty regard future housing. She has family concerns - her mother is terminally ill.

The lady has a history of breast cancer - mastectomy and twisted intestines. Suffering from ulcerative colitis: chronic inflammatory bowel disease (IBD) that primarily affects the large intestine (colon and rectum). It causes inflammation and ulcers in the lining of the colon and rectum, leading to symptoms like abdominal pain, bloody diarrhoea, and urgent bowel movements which she suffers from.

The lady would like to improve community access, finish setting up her home, she wants to shop in store x1 week over online all the time. She feels house bound which is not good for her physical and mental health problems. She is not comfortable relying on her daughters as they have small children . She has hopes to being able to explore access community resources for inclusion. She has sourced a PA, but this ended when she moved from BB as the PA does not over her area.

The lady is supported by Community Mental Health Services.
PA – hours required: 6 hours a week, s hours each time:
Days: Wed, Fri, Sun.
Qualities require in female PA:
• PA will need to be able to drive
• Be gentle and caring
• Not discuss their own personal history of difficulties
• Will need an introductory visit.
