Caring person to check in on a dementia person

I am looking for someone to help with the support of my father who is 96yrs old and suffering from dementia.
He has just been in hospital for the last 2 weeks and he is returning home. He was always self-sufficient before going into hospital so I don’t really know what would be involved for you to do when visiting. I think it will be an evolving care provision.
I would initially need someone to check on him in the mornings to make sure he is up and taken any medication and then again at lunchtime to again make sure he has eaten (lunch will have been made for him). I will be home about 4pm so I wouldn’t need anyone in the evening to come in. It would probably be a 2hr a day visit. It would also involve someone on a Saturday evening to come and make sure he has eaten his dinner and that he has washed and got ready for bed this would probably be an hour or two visit.
