Support is to enable a gentleman who is in a residential home to be able to go out into the community support can be delivered either as 2x two hour sessions or 1x four hour sessions.
Likes to go to the pub or to a cafe.
Has dementia and can present with signs of this, PA will need to ensure they have the care home's telephone number so they can phone for support if required.
PA who has the knowledge and skills to work with someone who has advance dementia will be required.
PA will require a car as he walks with a zimmer frame.
None smoker.
