About me/ support required: I am a 61-year-woman who is currently living in a Care Home in Newport where I have been receiving support to meet my care and support needs and health needs. I have a diagnosis of mild Learning Disabilities along with health conditions, so I need support every day; however, most of my support is provided by the care staff in my home. My mobility isn’t great needing assistance of 2 with transfers however, I would only be asking a personal assistant to help me access the community using my wheelchair or doing activities in my home such as pampering sessions. I have a good sense of humour and enjoy socialising as this helps with my mental wellbeing.

Additional Information: I have been diagnosed with the following health conditions which are managed by my care home staff team: Diabetes Mellitus Type 2, Hepatic encephalopathy; Acute and subacute hepatic failure without coma; Acute on chronic liver failure. Non-alcoholic liver cirrhosis, Thrombocytopenia, Iron deficiency anaemia, Congenital malformation syndromes predominantly affecting facial appearance - here Pierre Robin syndrome.
I need reminding and encouragement to drink plenty throughout the day and eat a good balanced diet if accessing the community. For my toileting needs, I do wear incontinence aids; however, I prefer to use my commode as much as possible.
