Older lady who is currently in a residential home. Has a diagnosis of dementia but is aware she is in a home but lacks the insight to understand why, believing she should be able to return home. Requires support to to help her access the community.

This can be broken up into 2 x 2 hours or 1 block of 4 hours. This may need to be done incrementally.
